Description
- Conduct thorough evaluations of patients in inpatient or outpatient settings and craft personalized treatment plans
- Implement specialized physical therapy interventions focusing on mobility, strength, balance, and overall functional improvement
- Collaborate closely with nurses, physicians, case managers, and other rehabilitation professionals to enhance patient outcomes
- Educate patients and their families on therapeutic objectives, discharge processes, and at-home exercise routines
- Accurately document patient evaluations, treatments, and progress in compliance with both facility and regulatory standards
- Participate in interdisciplinary conferences and care team rounds to ensure seamless care coordination
Education
- Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution
Certifications
- State licensure as a Physical Therapist
- CPR certification preferred
