Evolution Physical Therapy, Inc. is a privately owned physical therapy practice specializing in orthopedic outpatient care. Operating from a modern and expansive 5,000 square feet sports medicine facility, we pride ourselves on utilizing state-of-the-art technology and methodologies in rehabilitation. Our practice is dedicated to delivering high-quality care primarily for patients with private medical insurance, ensuring they receive the individualized support they need throughout their recovery journey.

This position requires working onsite within our home care setting, where you will interact directly with patients in a structured yet flexible environment. Our vibrant team consists of two experienced physical therapists and a physical therapist assistant (PTA), all committed to fostering collaboration and promoting the best therapeutic practices. Operating primarily during weekdays, your work schedule will be Monday to Friday from 9 AM to 5 PM, allowing for a balanced professional life.

• Assess and evaluate patients' physical conditions to understand their goals and needs, ensuring a personalized treatment approach.
• Manage a patient caseload effectively, treating one patient per hour to provide focused and quality care.
• Utilize evidence-based practice to develop comprehensive care plans that include various physical therapy techniques tailored to each patient's unique requirements.
• Implement cost-effective treatment modalities to enhance patients' mobility and functional independence while minimizing reliance on medications and invasive procedures.
• Create individualized fitness and wellness programs that promote healthy lifestyles, incorporating exercises designed to strengthen, increase flexibility, balance, and coordination.
• Motivate and guide patients throughout their treatment processes to ensure their engagement and commitment to rehabilitation.
• Maintain thorough documentation of all patient care services provided, including treatment outcomes and progress, to ensure continuity of care and compliance with regulatory standards.
• Collaborate with multidisciplinary healthcare professionals to provide comprehensive care, offering the best outcomes for our patients.

• A Doctorate in Physical Therapy (DPT) from an accredited program.
• A valid state license to practice as a Physical Therapist in New Jersey.
• Proven track record of maintaining a clean professional record, free from any board or disciplinary actions.
• Strong proficiency in manual therapy techniques and an exceptional ability to design and implement tailored exercise protocols.
• Competence in utilizing EMR software, specifically WebPT, for efficient patient management and record keeping.

• Experience working in orthopedic outpatient settings, with hands-on familiarity in managing surgical and non-surgical cases.
• A background in treating a diverse range of conditions with an outcome-focused approach.
• A commitment to ongoing learning and professional development in physical therapy techniques and methodologies.
