Position Overview

We are seeking a passionate and committed Licensed Clinical Social Worker (LCSW) to join our dedicated team at Bliss Reformation and Wellness Center. This full-time, remote position will play a vital role in supporting the psychosocial needs of our clients, primarily within a hospice care setting. You will contribute significantly to our goal of empowering seniors and their families to achieve emotional well-being and maintain quality of life during challenging times.

Work Environment

This position offers the flexibility of remote work, allowing you to interact with clients through virtual platforms while maintaining the essential personal connection that is vital in social work. As a part of our hospice center, you will be an integral part of an interdisciplinary team that supports clients in their homes or care facilities. Although the primary work hours are Monday to Friday from 8:00 AM to 4:30 PM, occasional evening or weekend hours may be required based on client needs. You will also need to be flexible in your schedule to accommodate visits and ensure seamless support for your clients.

Key Responsibilities
  • Conduct comprehensive assessments of clients' psychosocial needs, taking into account emotional, mental, and social factors impacting their well-being.
  • Collaborate with clients and their families to develop individualized care plans, addressing specific goals and preferences related to their overall health and autonomy.
  • Provide empathetic counseling and emotional support, aiding seniors and their families in navigating significant life transitions and managing challenges associated with aging and serious illness.
  • Advocate for clients' rights, ensuring access to community resources, healthcare services, and social support networks tailored to their needs.
  • Work closely within interdisciplinary teams, which may include nurses, caregivers, and other healthcare professionals, to facilitate comprehensive care coordination.
  • Regularly visit clients to monitor their well-being and living conditions, promptly addressing any safety concerns or issues that arise.
  • Encourage effective communication and conflict resolution among family members, enhancing the support system for the senior client.
  • Provide education to clients and families on available resources, support groups, and available programs to encourage social engagement.
  • Keep meticulous records of client assessments, care plans, progress notes, and any pertinent interactions, ensuring compliance with ethical standards and best practices.
  • Stay current on evolving social work practices, policies, and regulations pertinent to geriatric care.
Patient Population / Client Focus

You will primarily work with elderly clients facing a variety of psychosocial challenges associated with aging, chronic illnesses, and end-of-life transitions. A central focus of this role is to support clients in a hospice setting, ensuring dignity, respect, and emotional support throughout their care journey.

Qualifications
  • Earned a masterโ€™s degree in Social Work from an accredited institution.
  • Possession of current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
  • A minimum of two years of professional experience in a social work role, preferably within a geriatric or community-centered environment.
  • Extensive knowledge of social work principles, aging-related issues, and available community resources.
  • Demonstrated proficiency in assessment, counseling, and problem-solving skills.
  • Exceptional interpersonal skills, with an ability to empathize and form rapport with displaced individuals and families.
  • Adept at utilizing electronic health record (EHR) systems and technology solutions for client management.
Preferred Experience
  • Previous experience in hospice, palliative care, or geriatric social work settings.
  • Familiarity with community resources tailored for the elderly population and their families.
  • Established skills in conflict resolution and family mediation.
