Description
  • Conduct evaluations for patients in inpatient or outpatient settings and create personalized treatment plans
  • Implement physical therapy interventions targeting mobility, strength, balance, and overall functionality
  • Work in partnership with nurses, physicians, case managers, and other rehabilitation professionals to maximize patient outcomes
  • Instruct patients and their families on therapeutic objectives, discharge planning, and at-home exercise routines
  • Maintain detailed records of evaluations, treatments, and patient progress according to facility standards and regulations
  • Engage in care team discussions and interdisciplinary meetings to ensure comprehensive care coordination
Education
  • Doctor of Physical Therapy (DPT) or equivalent degree from a recognized institution
Certifications
  • State licensure as a Physical Therapist
  • Preferred CPR certification
  • Specialty certifications (e.g., in neurology or geriatrics) are advantageous but not mandatory
Skills
  • Preferred previous experience in a relevant clinical setting
  • Ability to assist patients with intricate medical and mobility challenges
  • Strong communication and collaborative skills
