Role Overview
The Case Associate provides administrative and clerical support to the care coordination process, including managing client intake and referrals, maintaining records and billing data, responding to inquiries, supporting assessments, and collaborating with internal teams and external partners to ensure efficient service delivery.

Job Responsibilities
  • Provide administrative support to care coordination process overall; prepare and maintain program data and billing; monitor and respond to client inquiries, concerns and complaints.
  • Responsible for ntake, processing of client referrals, and preparation of client files. Ensure timely follow-up to messages/inquiries made to care coordination team. Serve in a clerical capacity to review and monitor files and related documentation. Complete assigned billing tasks including data entry and maintenance. Provide pre and post assessment activities
  • Work with external partners to resolve billing matters. Attend all trainings as required. Provide support to clients, care coordinator, supervisors and management staff.
  • Work collaboratively with internal stakeholders and team members as needed.
  • Engage in ongoing learning, training, and skill development, seeking and applying feedback to continuously improve performance and contribute to team and organizational success.

PLEASE NOTE: Essential functions include all other duties and responsibilities as assigned.
  • Kneel and move from sitting, bending, kneeling, or standing multiple times a day
  • Climb up and down up to 3 flights of stairs at a time
  • Lift, push, and pull up to 25 lbs

Education and Experience Requirements:
Minimum
  • H.S. Diploma or General Education Degree (GED) Entry level
  • Strong communication, problem solving skills and critical thinking skills needed
  • Previous data entry skills are required.
Preferred
  • Associate's Degree (AA) in a related field of study 1-3 years relevant experience
