About the Float Nurse Practitioner Position 

Upperline Health is seeking a Float Nurse Practitioner to support home-based care services across designated markets. This role serves as a clinical resource, providing chronic disease management for high-risk Medicare patients, supporting market growth initiatives, leading patient engagement efforts, and ensuring continuity of care for our highest-risk patients. 

The Float NP will deliver comprehensive in-home assessments and follow-up visits, collaborate closely with physicians and interdisciplinary team members, and support the development and execution of individualized care plans. This position is ideal for clinicians who enjoy variety, autonomy, travel within a regional market, and caring for medically complex patients in their homes. In addition to delivering home-based care, the Float NP supports patient outreach and care gap closure efforts while providing coverage for in-clinic and telehealth NPs as needed. This role helps ensure continuity of care, timely patient access, and achievement of value-based care objectives across assigned markets.   The ideal candidate is adaptable, highly organized, thrives in dynamic environments, and is passionate about improving outcomes through value-based care. 

Deliver Home-Based Care: Conduct comprehensive medical assessments, annual wellness visits, follow-up evaluations, and urgent clinical visits in patients' homes. 

Ensure Care Continuity: Provide clinical coverage across multiple markets and patient panels to maintain seamless patient care during provider transitions, leaves, or vacancies. 

Manage Complex Chronic Conditions: Develop and implement individualized care plans for patients with chronic illnesses such as diabetes, peripheral vascular disease, heart failure, COPD, chronic wounds, dementia, and other high-risk conditions. 

Provide Basic Foot & Wound Care: Following completion of Upperline Health training and demonstrated competency, provide routine foot care services such as routine nail care as well as basic wound assessments and wound care interventions within the NP scope of practice. Collaborate closely with podiatrists, vascular specialists, and wound care teams to ensure timely treatment and escalation of complex conditions. 

Support Value-Based Outcomes: Identify clinical and social barriers to health, address care gaps, and implement interventions designed to reduce emergency room visits, hospital admissions, and disease complications. 

Collaborate Across the Care Continuum: Partner with physicians, care navigators, pharmacists, social workers, specialists, and community providers to coordinate comprehensive patient care. 

Educate Patients and Families: Provide guidance on medication management, disease prevention, nutrition, treatment adherence, safety planning, and care goals. 

Promote Accurate Documentation: Maintain timely and accurate medical documentation and coding practices consistent with risk adjustment standards, company policies, and regulatory requirements. 

Provide Flexible Clinical Coverage when necessary: Serve as a versatile clinical resource by supporting patient engagement efforts, assisting with outreach and care gap closure initiatives, and providing coverage for in-clinic and telehealth NPs as needed to ensure uninterrupted access to high-quality patient care across assigned markets.
