Psychiatric Mental Health Nurse Practitioner (PMHNP)

We are looking for a compassionate and committed Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our growing team. We partner with nursing homes throughout Rochester NY, delivering high-quality behavioral health services to older adults. This role is ideal for a clinician dedicated to geriatric psychiatry and passionate about enhancing the emotional and psychological well-being of senior residents.

Key Duties

  • Perform thorough psychiatric assessments and mental health evaluations for residents in long-term care facilities.
  • Identify psychiatric conditions and create customized treatment plans that meet each patient's specific needs.
  • Manage psychiatric medications, monitor progress, and adjust interventions when clinically appropriate.
  • Work closely with an interdisciplinary teamโ€”including physicians, psychologists, nurses, and social workersโ€”to ensure coordinated, comprehensive care.
  • Provide therapeutic support, including counseling and psychotherapy when needed.
  • Educate patients and families regarding diagnoses, treatment options, medication use, and strategies to support mental wellness.
