Job description

DESCRIPTION
We are currently seeking a friendly and compassionate Nephrology Nurse Practitioner to join Houston Nephrology Group. This position will require traveling to multiple outpatient dialysis offices throughout the Houston area.
Full Time - Monday - Friday, 40 hours per week
• Conduct dialysis rounds in multiple dialysis units.
• Comprehensive access assessment and management.
• Prescribe and adjust dialysis-related medications as needed.
• Assess and set parameters including target weight and dialysis prescriptions.
• Perform history and physical exams on new and established dialysis and AKI patients.
• Initiate appropriate dialysis orders and treatment and testing required for evaluation of illness.
• Order therapeutic and diagnostic procedures under the scope of training as directed by the supervising physician.
• Provide follow up and health maintenance care to dialysis patients in accordance with established protocols or specific instructions from the supervising physician.
• Provide counseling and instruction to patients regarding health related issues.
• Document clinical care of the patient according to established guidelines.
• Communicate with patients and family members.
• Consult with the supervising physician as needed.
• Outpatient office/clinic visits for diagnosis, treatment and prevention of renal disease and injury of CKD patients.
