Job Descriptionย 

We are seeking a dedicated LPN/RN to provide safe, effective, and compassionate nursing care to residents within a long-term care setting. This role involves assessing resident needs, delivering individualized care, and ensuring all nursing practices align with state and federal regulations. The RN willย be responsible forย overseeing andย modifyingย comprehensive plans of care and providing independent supervision within the nursing unit.ย 

Responsibilitiesย 

  • Assess residentsโ€™ needs andย monitorย changes in conditionย 

  • Develop, implement, andย modifyย individualized plans of careย 

  • Provide direct nursing care and administer treatments as orderedย 

  • Supervise CNAs and other personnel involved in resident careย 

  • Ensure compliance with state and federal regulations and facility policiesย 

  • Maintainย accurateย documentation and support quality care standardsย 

  • Promote a safe and supportive environment for residentsย 

Qualificationsย 

  • At least two years of experience in Skilled Nursing/LTC/Rehabย 

  • Current Licensed Practical Nurse (LPN)ย or Registered Nurse (RN)ย license in Oregon (OR)ย 

  • BLSย requiredย
