  • Provide skilled care to our patients allowing them to return to independence.
  • Conduct and complete initial assessment of our patients in order to generate a personalized plan of care in collaboration with physicians and the interdisciplinary team.
  • Educate our patients and their families on the next steps in continuing care and improvement of day-to-day life.
  • Coordinate and perform skilled treatments to enhance the well-being of the patient with the goal of enhancing patient health.
Qualifications
  • Valid registered nursing license (required)
