Job Summary: We are seeking a friendly and compassionate Infectious Disease Nurse Practitioner/ Physician Assistant to join our growing team. In this role, you will provide comprehensive pulmonary care to patients in post-acute care settings while collaborating closely with physicians, nursing staff, and interdisciplinary care teams. This is an exciting opportunity to make a meaningful impact while working in a supportive and collaborative clinical environment focused on improving respiratory outcomes and enhancing patients’ quality of life.

Responsibilities:

  • Provide infectious disease–focused patient care in poste-acute care settings
  • Conduct comprehensive clinical assessments with emphasis on infection-related conditions
  • Evaluate, diagnose, and manage acute and chronic infectious diseases, including respiratory, urinary, skin/soft tissue, and bloodstream infections
  • Develop and implement individualized treatment plans aligned with evidence-based infectious disease guidelines
  • Prescribe and manage antimicrobial therapies in accordance with state regulations and antimicrobial stewardship principles
  • Support infection prevention and control initiatives within the facility
  • Collaborate with interdisciplinary teams
  • Monitor patient progress and adjust care plans as needed
  • Maintain accurate and timely EMR documentation
  • Support initiatives focused on reducing hospitalizations and Return-to-Hospital (RTH) events

Requirements

  • Preferred a minimum of 6 months of experience in post-acute care setting
  • Preferred experience in Infectious Disease: training provided
  • Active and unrestricted state Nurse Practitioner license or Active PA license and NCCPA certification required
  • Master’s Degree in Nursing from an accredited Nurse Practitioner program or Master’s Degree from an accredited Physician Assistant program required.
  • CPR BCLS/BLS certified
  • Strong communication, organizational, and teamwork skills
  • Ability to work independently in the clinical environment
