Job description

Title of Position: Family Nurse Practitioner Location: Gateways Hospital - Nursing Exempt/Non-Exempt: Non-Exempt Union/ Non-Union: Non-Union Supervisor: Senior Director of Clinical Services
Gateways Hospital and Mental Health Center, located in Los Angeles, is a non-profit organization that provides an array of comprehensive mental health services for Children, Adolescents, Transitional Aged Youth, and Adults suffering from mental illness in the Greater Los Angeles area. The organization consists of a standalone fifty-five (55) )-bed hospital and six outlying outpatient facilities that have anywhere from fourteen (14) beds to one hundred and thirty-four (134) beds. The organization has approximately four hundred (400) employees.
SUMMARY OF POSITION
The Family Nurse Practitioner is a licensed practitioner under the supervision of a MD/DO, who is responsible for managing health related conditions and coordinating health care treatment through assessment, diagnosis, development of plan of care, implementation of treatment plan and evaluation of patient status.
This is a part-time role with a working shift of Sunday through Tuesday, 8hr or 6hr shifts
ESSENTIALS DUTIES
  • Obtain historical information about patient through records and patient self-reporting
  • Perform physical assessment/examination both routine, irregular and follow-up
  • Order and interpret diagnostic tests- labs, radiology, biopsy
  • Provide screenings and interventions relative to age
  • Diagnose and treat common medical conditions
  • Prescribe/Furnish medications
  • Perform individual and group patient education
  • Complete documentation of patient information, visit history, necessary referrals, lab follow-up, phone calls, and consults in EMR.
  • Use of computer, software programs (including EMR), printer, scanner, and communication with billing
  • Communicate effectively with nursing, social work, psychology, nutrition, and pharmacy towards maintaining an interdisciplinary model of care.
  • Perform non-clinical tasks as required (e.g. reporting notifiable diseases, patient vaccination, temperature management, maintain clinic appearance)
  • Travel to other local Gateways facilities as needed
  • Chart all patient transactions in accordance with policies and procedures. Write orders and record progress notes which will be reviewed and countersigned by supervising physician;
  • Communicate and work with medical staff to ensure coordinated efforts for the provision of high quality medical treatment and patient service;
  • Assist management in identifying, analyzing, and resolving work problems;
  • Evaluate the effectiveness of existing clinical methods and procedures and suggest improvements;
  • Perform daily rounds as required and documents patient care orders;
  • Prepare timely, legible, and complete documentation of all patient care as provided by law, regulation, and established policy;
  • Other duties may be assigned or may be modified as business needs dictate.

EDUCATION & CERTIFICATES
  • MSN from an accredited Family Nurse Practitioner program
  • Current National Board Certification (AANP or ANCC)
  • California license to practice as a Nurse Practitioner
  • California Nurse Practitioner Furnishing number

EXPERIENCE/QUALIFICATIONS
  • Passion for Gateways Hospital and Mental Health Center concept and vision
  • Minimum of one year of medically relevant experience or equivalent
  • Ability to collaborate with professional colleagues to provide quality care through an interdisciplinary model
  • Ability to consistently display respect and maintain appropriate boundaries with patients who have been hospitalized for mental health treatment.
  • Excellent verbal, written and interpersonal communication skills.
  • Bilingual Preferred (English/Spanish)
  • NextGen Knowledge (Preferred)
  • Excellent Computer Skills
  • Demonstrated ability to work both independently and in a team environment.

REQUIREMENTS
  • Must pass Department of Justice (DOJ), Federal Bureau of Investigations (FBI), and Community Care Licensing (CCL) background clearance.
  • Valid California Driver's license.
  • TB clearance.
  • Driving record acceptable for coverage by Gateways insurance carrier.
