Child & Adolescent Intensive Care Coordination Case ManagerAnnual Salary - $53,569Work Schedule: Monday - Friday 8:30 am - 5:00 pm (Some on-call responsibilities)Case Management is a vital part of the H-NNCSB service delivery system, serving as a unifying link between families, children, and the supports they need. The Intensive Care Coordination (ICC) Case Manager offers a higher level of support, including clinical assessments and level-of-care determinations, for individuals diagnosed with a Serious Emotional Disturbance (SED) or an Intellectual or Developmental Disability (IDD).
