Position Summary

The Medical Coordinator Assistant is a delegated clinical support role responsible for assisting with client health support, care coordination, and documentation for clients enrolled in Whole Person Care Clinic programs, including Recuperative Care. This role operates only under the direction and supervision of authorized clinical staff and supports health stabilization, recovery, and continuity of care within scope of practice per program guidelines.

The Medical Coordinator Assistant supports CalAIM-covered services in a non-independent capacity and escalates all clinical concerns, changes in condition, or scope-related questions in accordance with WPCC policy and applicable law.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Client Health Support & Monitoring (Delegated, Non-Independent)

  • Conduct daily non-clinical wellbeing checks and assist with intake processes under authorized supervision
  • Observe and report changes in client condition to supervising staff
  • Provide basic health education related to hygiene, wellness routines, and medication adherence without clinical interpretation
  • Support a safe, trauma-informed environment for clients recovering from illness or injury

Medication Support (Delegated)

  • Support client self-administration of medications in accordance with WPCC procedures and authorized delegation
  • Monitor adherence to medication routines and report concerns promptly
  • Maintain accurate and timely medication logs and related documentation
  • Escalate medication errors, refusals, adverse reactions, or discrepancies immediately

Care Coordination Support

  • Assist with scheduling and coordinating medical, behavioral health, and specialty appointments
  • Support follow-up care activities and referrals under authorized direction
  • Participate in daily huddles and interdisciplinary team meetings as assigned
  • Collaborate with Case Managers, behavioral health partners, housing staff, and clinical staff to support integrated care

Documentation & Compliance

  • Document health-related interactions, medication support, and coordination activities in the EHR accurately and timely
  • Maintain documentation in compliance with HIPAA, CMIA, 42 CFR Part 2, OSHA/Cal-OSHA, and WPCC policies
  • Support tracking of program metrics and client health outcomes as directed

Program & Safety Support

  • Maintain clean, safe client care areas and support infection-control procedures
  • Assist with basic facility and supply support related to client care operations
  • Participate in required training, competency signoffs, and onboarding activities

Qualifications and Requirements

Required

  • High school diploma or equivalent
  • Current EMT Certification
  • Meet credentialing and certification requirements as defined by WPCC program guidelines
  • Basic Life Support (BLS) certification

Preferred

  • Minimum one (1) year of experience in a clinical, shelter, or social services setting
  • Experience working with individuals experiencing homelessness, chronic illness, or behavioral health challenges
  • Familiarity with electronic health record systems

  • Exposure to clients with communicable conditions or behavioral health needs
  • Use of PPE and compliance with OSHA/Cal-OSHA and WPCC safety standards

SCHEDULE REQUIREMENTS

  • Full-time (40 hours/week), Variable shifts
  • Must be available for assigned shifts (including nights/weekends)
