Description:
Position Purpose
Mercy Care Atlanta is seeking a compassionate and experienced Street Team Manager to provide leadership, coordination, and operational oversight for our multidisciplinary Street Team serving individuals experiencing unsheltered homelessness. This position leads outreach staff, PATH services, and coordination with Street Medicine clinicians to ensure high-quality, trauma-informed care for vulnerable populations. The Manager plays a critical role in advancing Mercy Care's mission by developing strategic outreach efforts, fostering community partnerships, and ensuring compliance with grant and program requirements.
What You Will Do
  • Lead all Street Team operations, outreach activities, and PATH program services.
  • Oversee coordination between outreach staff and Street Medicine providers.
  • Develop outreach schedules and deploy services to encampments and high-need areas.
  • Provide direct field-based engagement and support complex client cases.
  • Supervise, mentor, and develop all social care outreach staff
  • Lead hiring, onboarding, training, performance management, and staff development initiatives.
  • Collaborate with Mercy Care clinical leadership, the Chief Medical Officer, and community partners.
  • Monitor program outcomes using HMIS, EPIC, and other reporting systems.
  • Prepare grant reports, support funding proposals, and ensure compliance with all regulatory requirements.
  • Drive quality improvement efforts through data analysis, performance monitoring, and strategic planning.

Minimum Qualifications
  • Bachelor's degree required; Master's degree preferred.
  • Minimum of seven (7) years of experience in homelessness services, social services, behavioral health, community health, EMS/paramedic services, public health, or related human services field.
  • Three to five (3-5) years of management or supervisory experience required.
  • Experience with street outreach preferred.
  • Experience using data management systems such as HMIS, EPIC, or similar platforms.
  • Strong leadership, communication, organizational, and problem-solving skills.
