As part of a remote team, the LCSW will leverage technology to ensure confidential and efficient service delivery, maintaining compliance with healthcare regulations and protocols.

• Establish immediate rapport with patients and their families by employing active listening and clear communication techniques, avoiding jargon to ensure understanding and comfort.
• Collaborate effectively within a professional team that includes medical providers, behavioral health specialists, and family members to formulate comprehensive care plans.
• Conduct brief but thorough assessments and screenings for various mental health conditions, such as depression, anxiety, and substance abuse, and refer patients appropriately for further evaluation as necessary.
• Manage your own caseload by providing follow-up interventions tailored to patient needs, thus fostering ongoing support and engagement in their recovery journey.
• Utilize electronic medical records (EMR) diligently, ensuring that all documentation of patient encounters is completed within 48 hours to support integrated healthcare delivery.
• Create and implement care plans that incorporate necessary support services, ensuring proper communication among all stakeholders involved in patient care.
• Safeguard patient confidentiality and privacy throughout all interactions, documentation, and data handling.
• Actively participate in team supervision, case conferences, and quality improvement meetings to enhance care practices and outcomes.
• Assist in referring patients who require psychiatric medication evaluations to the appropriate medical providers.
• Respond promptly to calls from patients, family members, and treatment providers regarding appointments and concerns.
• Identify community resources and support services that align with the patients’ goals, helping them efficiently connect with available options.
• Perform additional duties as assigned to contribute to the overall mission of the detox center.

This role primarily involves working with individuals who are undergoing detoxification and require immediate support in managing both substance use and mental health challenges. The LCSW will engage with a diverse patient population, often from underserved backgrounds, ensuring culturally competent care tailored to meet their unique needs and circumstances.

• Master’s degree in a Behavioral Health field.
• Valid Licensed Clinical Social Worker (LCSW) credential is mandatory.
• Certification in CPR or BLS.
• Minimum of three years of direct experience in the behavioral health field, focusing on substance use and mental health care.
• Demonstrated cultural competency and experience working with underserved populations, including individuals experiencing homelessness.
• Familiarity with trauma-informed care practices and social justice principles.
• Proficiency in computer skills for data management and documentation using EMR systems.
• Bilingual proficiency in English and Spanish is preferred.

• Experience using evidence-based therapies and cognitive-behavioral interventions within a primary care setting.
• Background in community resource linkage and crisis intervention strategies is advantageous.
• Knowledge of population health principles and experience in preventive care programming.
