Responsibilities

1. Assure the safety of each resident when transporting to and from the doctor's office and/or hospital.2. Accompany each resident on all medical visits. Stay with resident(s) until visit is complete.3. Observe the following guidelines at each visit:ย  ย  ย a. Sign in resident upon entrance.ย  ย  ย b. Present resident's documents to office personnel.4. Before leaving doctor's office, please observe the following:ย  ย  ย a. Obtain resident's documents.ย  ย  ย b. Obtain any new prescriptions.ย  ย  ย c. Check to make sure doctor has written on the doctor's order form.ย  ย  ย d. If follow-up appointment necessary, obtain appointment cards.5. After returning to facility, report to RCM and be prepared to answer questions concerning visit.6. Performs other duties as required and assigned.

Qualifications
  • ย Physically able to help ambulatory and non-ambulatory residents transfer to and from wheelchair to vehicle.
  • Possess a valid state driver's license.
  • Qualified as a Certified Nurse Assistant (C.N.A.).
  • Clean driving record.
  • Clean criminal record.
  • Any other state required certifications
