Job description

Schedule: Full-time| Monday - Friday | 8:00 a.m. to 4:30 p.m.
Pay Range: $72,000 - $90,000 (salaried exempt), based on experience, qualifications, and location. This position also includes unlimited incentive bonuses.

As a Home Health Liaison, you will play a critical role in driving referral growth and expanding Vivie's presence in the community. You'll act as an advocate for home healthcare and hospice services, working directly with hospital systems, clinics, senior living communities, and other referral sources. Through relationship-building, strategic planning, and expert education, you'll ensure that Vivie's services reach those who need them most.
As a Home Health Liaison, you will:
  • Develop and Manage Referral Partnerships - Build and maintain strong relationships with healthcare providers, social workers, and senior living communities to drive referrals and achieve census goals.
  • Drive Sales Strategy and Market Growth - Identify new opportunities for growth using referral and market share data, develop and execute territory sales plans, and track progress using CRM tools.
  • Serve as an Expert Resource - Educate referral partners and community members on Vivie's home healthcare and hospice services, collaborating with Nurse Navigators, marketing, and clinical teams.
  • Deliver Exceptional Customer Service - Ensure strong account management, resolve challenges, and provide first-class service to maintain positive relationships with referral sources.
  • Ensure Compliance and Industry Awareness - Stay informed on healthcare operations, legal guidelines, competitive analysis, and marketplace trends to navigate changes and remain a trusted partner.
  • Other Duties as Assigned - The duties and responsibilities listed above are representative of the nature and level of work assigned and are not necessarily all inclusive.

This job also requires:
  • Associate's degree required. Bachelor's degree preferred.
  • Sales experience in health-related field, preferably within a home health or hospice setting.
  • Proven track record in sales, business development, or healthcare marketing.
  • Strategic thinking and data-driven decision making to identify and capitalize on growth opportunities.
  • Experience with CRM tools to document and track sales activity.
  • Valid driver's license and insured personal vehicle.
  • Ability to pass state mandated background check.
  • Physical capability to perform all essential job functions.
