This full-time position will involve working closely with patients in their homes in Front Royal, Virginia, where you will assess, plan, and implement effective rehabilitation strategies to enhance mobility and independence. Key Responsibilities
  • Design, implement, and oversee personalized physical therapy treatment plans aimed at preventing disability, alleviating pain, and restoring optimal muscle function, in line with assessed patient capabilities.
  • Supervise and mentor Physical Therapy Assistants, ensuring adherence to treatment protocols and fostering continuous learning and improvement.
  • Conduct comprehensive evaluations using diagnostic and prognostic tests related to muscle, nerve, and joint functions, providing critical insights for tailored rehabilitation strategies.
  • Assess patients’ home environments to make recommendations that enhance therapy outcomes, ensuring that clients can live as independently as possible.
  • Administer manual and therapeutic exercises, instructing patients in both independent and guided physical activities to support recovery.
  • Utilize EMR software, particularly Web PT, for thorough documentation of evaluations, progress notes, and discharge summaries, ensuring compliance with organizational policies.
  • Participate actively in interdisciplinary team meetings, collaborating with physicians, case managers, and other healthcare professionals to review and adapt patient care plans as necessary.
  • Document and report any changes in patient conditions, communicating effectively with physicians and family members to modify treatment strategies.
  • Provide instructional sessions for patients and their families on rehabilitation techniques and the proper use of physical therapy devices.
  • Complete all assigned documentation, including orders and clinical records, in a timely manner to ensure coordinated care delivery.
