This position is based in the patient’s home, where you will conduct therapy sessions in a comfortable and familiar setting. You will work Monday through Friday, allowing for a stable work-life balance while maximizing your impact on patients’ lives. • Design, implement, and oversee personalized physical therapy treatment plans aimed at preventing disability, alleviating pain, and restoring optimal muscle function, in line with assessed patient capabilities.
• Supervise and mentor Physical Therapy Assistants, ensuring adherence to treatment protocols and fostering continuous learning and improvement.
• Conduct comprehensive evaluations using diagnostic and prognostic tests related to muscle, nerve, and joint functions, providing critical insights for tailored rehabilitation strategies.
• Assess patients’ home environments to make recommendations that enhance therapy outcomes, ensuring that clients can live as independently as possible.
• Administer manual and therapeutic exercises, instructing patients in both independent and guided physical activities to support recovery.
• Utilize EMR software, particularly Web PT, for thorough documentation of evaluations, progress notes, and discharge summaries, ensuring compliance with organizational policies.
• Participate actively in interdisciplinary team meetings, collaborating with physicians, case managers, and other healthcare professionals to review and adapt patient care plans as necessary.
• Document and report any changes in patient conditions, communicating effectively with physicians and family members to modify treatment strategies.
• Provide instructional sessions for patients and their families on rehabilitation techniques and the proper use of physical therapy devices.
• Complete all assigned documentation, including orders and clinical records, in a timely manner to ensure coordinated care delivery.

This role involves working with a diverse patient population that includes individuals recovering from surgery, those managing chronic conditions, or elderly clients requiring rehabilitation services. Your personalized approach will not only significantly impact their recovery trajectories but also promote overall wellness within the community. You will focus on enhancing functional mobility and independence while developing a trusting relationship with patients and their families throughout the healing process.

• A Doctorate in Physical Therapy (DPT) from an accredited program.
• An active Physical Therapist license in Virginia with a clean professional record free from sanctions or disciplinary actions.
• A solid foundation in manual therapy techniques and the ability to implement advanced therapeutic modalities.
• Demonstrated expertise in creating and executing effective exercise programs that yield positive patient outcomes.
• Proficient in using EMR software for accurate and timely documentation.
• Current knowledge of research-supported physical therapy practices and interventions.

• Experience in home health care or working in an on-site rehabilitation setting is highly desirable.
• Familiarity with OASIS documentation standards and requirements.
• Advanced certifications in specialty areas such as orthopedics or geriatrics.
