Job description

Location: Northern Dodge County Position Type: Full Time Schedule Requirements: Primarily day shift, but also includes a required on-call, weekend, and holiday rotation.Sign-On Bonus: This position is BONUS Eligible at 6 months of successful employment. Bonus range is $3,000 to $5,000 depending on hospice experience.

Purpose of the PositionRainbow Community Care is currently accepting applications for dedicated home hospice CNAs to work full time in the Northern Dodge County area. Use your skills to provide high quality home care to terminally ill patients. The position involves working in different working environments, including patients' homes and healthcare facilities.
Key Responsibilities
  • Assists with ambulation and transfers of patient.
  • Maintains a clean, safe and comfortable environment.
  • Prepares meals and assists patients with eating.
  • Gives emotional and psychological support to the patient/family.
  • Reports status/observations of assigned patients according to RCC guidelines.
  • Promotes patients' independence.
  • Maintains up-to-date patient records and documents care given and patient condition for each visit.
  • Assists the patient in activities of daily living including personal grooming, feeding, drinking, toileting, bathing, skin care, nail care, dressing, transferring and walking. Light housekeeping duties as assigned.
  • Other duties and projects as assigned.
Personal Qualifications
  • Maintains patient confidentiality.
  • Maintains all required licenses, certifications or registrations required of this position.
  • Drives to and from patients' homes and to meetings at other locations as necessary.
  • Able to be flexible and be included in weekend and holiday rotation schedule.
  • Able to maintain healthy boundaries.
  • Able to maintain positive interpersonal relationships.
  • Self-directed.
  • Sensitive to needs of others without being judgmental.
  • Tactful and courteous in contact and communication with others.
Professional Qualifications
  • Valid driver's license required. Current auto insurance required.
  • Certification as a Nursing Assistant and valid registration with the Wisconsin CNA Registry required.
  • Minimum of 6 months CNA experience preferred.
  • Hospice experience is preferred.
