Description
  • Assess patients in various settings, whether inpatient or outpatient, and craft personalized treatment plans
  • Deliver physical therapy interventions focusing on mobility, strength, balance, and overall functionality
  • Collaborate closely with nurses, physicians, case managers, and a team of rehab professionals to enhance patient outcomes
  • Educate patients and their families about therapeutic objectives, discharge planning, and home exercise routines
  • Keep meticulous records of evaluations, treatments, and progress in line with facility and regulatory standards
  • Engage actively in care team meetings and interdisciplinary conferences to ensure cohesive patient care
Education
  • Doctorate in Physical Therapy (DPT) or a recognized equivalent from an accredited institution
Certifications
  • Current state licensure as a Doctor of Physical Therapy
  • CPR certification is preferred
