Social Worker

Royal Oaks Nursing and Rehabilitation | Urbandale, IA

Schedule: Full-Time

Rate: Up to $34 based on experience

Royal Oaks Nursing and Rehabilitation is seeking a dedicated Social Worker to join the Social Services Department at our skilled nursing facility in Urbandale, IA. This role serves as a liaison and advocate for residents' social and emotional needs and is responsible for supporting the planning, implementation, and coordination of social services throughout the facility.

Position Overview

The Social Worker ensures that residents' medically related social and emotional needs are identified and addressed through comprehensive assessments, individualized care planning, and ongoing support. This position plays a vital role in resident adjustment, family communication, crisis intervention, discharge planning, and regulatory compliance.

Key Responsibilities

  • Develop comprehensive social assessments and individualized care plans with measurable goals and interventions.

  • Participate in pre-admission and admission processes.

  • Serve as a resource to residents and families during adjustment periods, crises, or special circumstances.

  • Document residents' behavioral and psychosocial status accurately and in a timely manner.

  • Provide individual and group support services to residents and families.

  • Establish and coordinate volunteer programs to support resident social service needs.

  • Orient residents and families to the facility and assist with transition and adjustment.

  • Collaborate with the interdisciplinary team to promote resident-centered care.

  • Ensure compliance with federal, state, and local regulations governing social services in long-term care.

Qualifications

  • Bachelor's Degree in Social Work or a related field required.

  • Previous long-term care (LTC) experience preferred.

  • Prior supervisory experience preferred.

  • Strong knowledge of social service regulations and best practices in healthcare.

  • Excellent communication, organizational, and interpersonal skills.

  • Ability to manage multiple priorities in a fast-paced environment.
