General Summary of Duties: The Director of Strategic Initiatives leads the development, alignment, and implementation of high-impact strategic and grant-funded initiatives that advance the organization's mission and population health strategy. This role partners with executive leadership and internal and external stakeholders to integrate organizational priorities, funding opportunities, and community partnerships into scalable, high-performing programs. In collaboration with the Director of Grant-Funded Programs, the Director ensures initiatives are strategically aligned, effectively executed, and achieve intended outcomes.
Essential Functions:
  • Lead the design and development of strategic programs and innovative care models aligned with organizational goals.
  • Translate leadership priorities and funding opportunities into scalable, sustainable initiatives.
  • Identify, develop, and integrate grant-funded programs and strategic partnerships.
  • Align funding streams, programs, and operations to create cohesive, high-performing systems.
  • Collaborate across leadership, clinical teams, operations, and community partners to drive shared goals.
  • Support the implementation, launch, and transition of new initiatives into ongoing operations.
  • Monitor program performance, analyze outcomes, and drive continuous improvement through data and feedback.
  • Build and strengthen relationships with community organizations, health systems, and strategic partners.
