Position Overview:
The RN is responsible for providing direct patient care and case management in accordance with physician orders. This includes conducting patient admissions, developing care plans, and coordinating care with the interdisciplinary team.


Key Responsibilities:
• Obtain medical history and assess patient condition and needs
• Conduct physical assessments, including vitals and mobility evaluations
• Educate patients and families on care plans and home health services
• Deliver skilled nursing care while maintaining HIPAA compliance
• Document and submit accurate visit records promptly
• Develop and update individualized care plans as needed
• Coordinate with LPNs, and Home Health Aides
• Facilitate referrals for additional services such as therapy or medical social work
• Evaluate the effectiveness of nursing services and adjust as appropriate

Qualifications:
• Active RN license in California
• Experience with OASIS and WellSky (Kinnser) strongly preferred
• Med-surg and wound care experience a plus
• Current physical exam (within last 6 months)
• Current TB test (within last 12 months) and CPR certification
• Valid driver’s license
• Must be able to attend orientation at our Yuba City office
