Job description

Description:

Location: Fully Remote (Possibly Hybrid if Open)

Preferred Residence: Washington, DC, Maryland, or Virginia

Schedule: Flexible

Make a Meaningful Impact Through Innovative, Whole-Person Care

At CloseKnit, we are redefining healthcare by integrating primary care, behavioral health, and technology to deliver accessible, patient-centered care. As a growing hybrid healthcare organization serving patients across Washington, DC, Maryland, and Virginia, we believe that mental health is a vital component of overall wellness. We are seeking compassionate, licensed Mental Health Therapists who are passionate about helping individuals navigate life's challenges and achieve meaningful, lasting change. In this role, you will provide evidence-based therapy to children, adolescents, and adults through a flexible hybrid care model that combines virtual care with opportunities for in-person collaboration and patient engagement.

As a valued member of our interdisciplinary care team, you will work alongside primary care providers and healthcare professionals to deliver coordinated, whole-person care that improves outcomes and expands access to behavioral health services. You'll have the opportunity to build strong therapeutic relationships, support diverse patient populations, and make a direct impact on the communities we serve. If you're looking for a role that offers flexibility, clinical autonomy, professional collaboration, and the opportunity to be part of an organization transforming healthcare delivery, we encourage you to apply.

Requirements:
  • Master's degree in Social Work, Counseling, Psychology, or a related behavioral health field.
  • Active independent clinical licensure in at least one of the following jurisdictions: Massachusetts (MA), Washington, DC, Maryland (MD), or Virginia (VA).
  • 3–5 years of clinical experience providing therapy to youth, adults, and individuals experiencing behavioral health challenges.
  • Experience treating patients across the lifespan.
  • Strong knowledge of evidence-based therapeutic modalities and interventions.
  • Excellent communication, relationship-building, documentation, and organizational skills.
  • Telehealth experience.

Preferred Qualifications

  • Experience working with specialized populations or treatment areas such as trauma, anxiety, depression, substance use disorders, or grief and loss.
  • Experience providing telehealth services.
  • Familiarity with Athena EMR and electronic health record documentation standards.
  • Experience working within integrated or collaborative care environments.

Other Requirements
Assessment & Evaluation:

  • Conduct comprehensive assessments of behavioral, emotional, and mental health needs.
  • Develop individualized treatment plans aligned with client goals.

Therapeutic Interventions:

  • Provide evidence-based individual, family, and group therapy using modalities such as CBT and DBT.
  • Deliver care both onsite and virtually in alignment with the hybrid care model.

Monitoring & Documentation:

  • Track client progress and adjust treatment plans as needed.
  • Maintain accurate and timely clinical documentation, including progress notes and treatment outcomes.

Collaboration & Communication:

  • Work collaboratively with Primary care physicians, psychiatrists, social workers, case managers, and other healthcare professionals.
  • Communicate treatment plans, progress, and recommendations with clients, families, and stakeholders.

Crisis Intervention:

  • Provide crisis intervention and support to clients in acute distress.
  • Develop safety plans and strategies to prevent or mitigate crises.

Education & Support:

  • Educate clients and families on mental health conditions, treatment options, and coping strategies.

Caseload Expectations:

  • Maintain a minimum caseload of 25+ clinical sessions per week.
  • Full-time 40 hours per week. Flexible scheduling options to include evenings and weekends.
