Job Title: Direct Care Professional / ADT Transportation Driver

Location: Brandon, FL

Employment Type: Full-Time/Part-Time

Reports To: Administrator, Program Director, Manager and Supervisor

Position Overview:

Destiny Quality Care, LLC is seeking responsible and passionate individuals to join our team in a dual-capacity role that combines direct care services with transportation support for individuals with physical and intellectual disabilities. This position requires unwavering commitment to client safety, dignity, and empowerment.

Minimum Qualifications:

•           Must be at least 18 years of age

•           Completion of the following trainings (or willingness to complete upon hire):

•           Zero Tolerance Training

•           Direct Care Competencies

•           HIPAA Compliance

•           HIV/AIDS Awareness

•           Bloodborne Pathogens

•           CPR Certification

•           Waiver Requirements Training

•           Must be able to perform all duties outlined in assigned Individual Service Plans (ISPs)

•           Possession of a valid Florida driver’s license, a clean driving record.

 Daily job performance

•           Flexibility to work evenings, weekends, and holidays as scheduled

•           Must submit to pre-employment, annual, random, and for-cause drug screenings

•           Must pass local and state background screenings

 Essential Functions

•           Provide personal care services in alignment with service plans, including:

•           Transferring, bathing, toileting, feeding

•           Housekeeping, errands, and emergency response

•           Ensure safe and timely transportation of clients to and from the ADT program and community outings

•           Observe and report changes in client condition to the Agency

•           Maintain professional and confidential communication with the Agency and clients’ families

•           Complete daily documentation of tasks and activities

•           Assist with menu planning and meal preparation as needed

•           Support individual rights and program planning

•           Assist with budgeting and spending guidance for clients

•           Perform other duties as assigned by supervisors, including priority care back-up

Education:

•           High school diploma or GED preferred

Knowledge, Skills, and Abilities:

•           Demonstrated ability to provide services to individuals with special needs using a positive, respectful

            approach.

•           Strong teamwork skills and ability to represent the Agency positively in the community

•           Patience, empathy, and effective communication skills

•           Basic math, reading, and writing proficiency

•           Commitment to professional ethics and confidentiality

•           Knowledge of and ability to comply with Federal, State, and Agency regulations
