This position offers an onsite work setting in the communities we serve, focusing primarily on home care. • Conduct thorough consultations with patients to assess their physical conditions, medical histories, and specific symptoms.
• Evaluate movement dysfunctions in patients and develop personalized treatment plans aimed at improving their mobility and overall physical well-being.
• Educate clients on therapeutic exercises, ensuring correct execution to maximize rehabilitation outcomes.
• Utilize techniques such as massage, stretching, and electrical stimulation to facilitate recovery and promote healing.
• Assist patients with mobility aids (e.g., wheelchairs, walkers) to enhance their independence and safety during treatment.
• Maintain detailed documentation of patient progress and treatment goals to track improvements and modify plans as necessary.
• Provide guidance to patients and their families on at-home treatment options and ongoing exercises.

In your role, you will work with a diverse group of patients ranging from children to seniors, each with unique diagnoses. Treatment may involve rehabilitation after surgeries, sports injuries, chronic pain management, or developmental challenges. By tailoring your approach to the individual needs of each patient, you will play a crucial role in enhancing their functional abilities and quality of life.

• A Doctor of Physical Therapy (DPT) degree from an accredited institution.
• A valid license to practice physical therapy in the state of Connecticut.
• Strong verbal and written communication skills for effective interactions with patients and healthcare teams.
• Proficiency in computer systems and software for patient records and documentation.
• A flexible and adaptable mindset to manage varying patient needs and schedules effectively.

• A minimum of two years in a clinical setting, demonstrating competency in various therapeutic techniques and approaches.
• Experience in home care or outpatient settings is a strong advantage.
