Social Worker
Location: Omaha, NE 68104
Job Type: Full-time 
Schedule: Monday-Friday, 9:00 AM-4:30 PM
Starting at $50,000 a year for candidates with a Social Worker degree, or $45,000 a year with a Social Services Designee credential, more with experience. Join Maple Crest Health Center in Omaha and support residents and families through some of the biggest transitions in their lives.
What You'll Do
- Conduct psychosocial assessments and counsel residents and families
- Support residents through admissions, transitions, and adjustment to aging or illness
- Collaborate with the interdisciplinary team on care planning and documentation, including MDS
- Assist with discharge planning and connect residents to community resources
- Advocate for residents and provide crisis intervention when needed
- Maintain accurate documentation and support financial and insurance processes
- Serve as a liaison between families, agencies, and mental health services
What You'll Need
Required (one of the following):
- Social Worker degree, or
- Social Services Designee certification
Also required:
- At least 1 year of interdisciplinary team experience
- Proficiency with computer systems and maintaining confidentiality
Preferred:
- Long-term care or hospice experience
Pay and Benefits
- $50,000/year starting for Social Worker degree holders, $45,000/year starting for Social Services Designee credential holders, more for candidates with relevant experience
