DEPARTMENT:SOCIAL SERVICE

GENERAL JOB DESCRIPTION:-Must be able to identify social and emotional needs; knowledgeable of methods toย ย use to meet those needs; and provide services to meet those needs. Provide medically-ย related social services to attain or maintain the highest practical physical, mental,ย and psycho-social well-being of each resident

JOB REQUIREMENTS:EDUCATION:-High school graduate, previous work experience in long term care

EXPERIENCE:-Bachelor's degree in social work, 2 years experience working in geriatrics,ย ย valid driver's licenses

ESSENTIAL JOB DUTIES:-Performs resident admission tasks such as: telephone or personal inquiries, tours,ย and admissionย  contracts if necessary-Assist residents transferring home or to another institution-Serves as the ombudsman for the facility's residents-Performs social service assessments on each resident from which and individualizedย Care Plan is written-Prepares social service progress notes quarterly on each resident-Coordinated outpatient appointments to include transportation-Assist residents in meeting their personal ne3eds either by contacting residentsย ย families or via a weekly shopping trip-Attends resident council meetings, care plan meetings, and other appropriateย ย committee meetings-Prepares bi-monthly newsletter article-Prepares an annual social services department budget-Performs additional duties as directed by the Administrato
