  • Minimum of 1 year experience working with the frail or elderly population is required (no new grads)
  • Not accepting visa candidates
  • Provider must be interested in Perm (not accepting Locums)
  • ALL OF THE FOLLOWING ARE REQUIRED AT NAME CLEAR UNLESS NOTED OTHERWISE.

    • Board Certified in Internal Medicine or Family Medicine - REQUIRED
    • Board Certified in Geriatric Medicine - PREFERRED
    • Active DE license - REQUIRED
    • CSR/CDS Certification (prior to start) - REQUIRED
    • DEA Registration - Verbal Confirmation (prior to start) - REQUIRED
    • Availability to begin working and state interest in Perm - REQUIRED
    • Minimum 1 year working with the frail or elderly population - REQUIRED
    • BLS (prior to start) - REQUIRED
    • Clean/Clear malpractice - HIGHLY PREFERRED
    • EMR TruChart experience - HIGHLY PREFERRED
