Job description

This Night Shift RN role is based in our inpatient Hospice House with 3-12 hour shifts, an every other weekend commitment, and rotating holidays. There is a $40,000 Sign On Bonus available for qualified candidates! Apply today! 

Job Summary:

Multiple areas utilize staff, including, but not limited to, the Inpatient Unit (IPU), Care 24 field, Care24 Triage and Continuous Home Care (CHC). In all roles, the RN utilizes the nursing process of assessment, planning, intervention, implementation and evaluation. The RN is skilled at assessment and management of pain and other complex symptoms frequently present at the end of life, including education and support to patient and family. The RN works with the interdisciplinary hospice team to plan and direct services to meet the needs of individuals and families.

Essential Job Functions:

  • All Hospice RN’s will cross train to other departments to meet the needs of the organization.
  • Provide nursing services, treatments, diagnostic and preventive procedures requiring the skills of a registered nurse according to policies, procedures and protocols.
  • Participate in interdisciplinary group meetings as needed.
  • Assists in development, implementation and revision of the interdisciplinary team care plan for each patient and family. Establishes goals, interventions including the patient and family in the process.
  • Assess patient and report to the attending physician and appropriate interdisciplinary team members, response to treatments and medications as well as changes in the patient’s physical or emotional condition.
  • Coordinate care with other interdisciplinary team members and with community resources as needed.
  • Teach, supervise and counsel the patient/family regarding pain and symptom management and psycho-social aspects of care for the terminally ill.
  • Assess and maintain patient safety.
  • Teach the patient and family/caregiver self-care techniques as appropriate. Provide medication, diet and other instructions as ordered by the physician and recognize and utilize opportunities for teaching identified needs with patients and families/caregivers.
  • Assist the patient and family/caregiver and other team members in providing continuity of care.
  • Supervise care provided by LPN’s and Home Health Aides.
  • Attend and participate in clinical education, in-service and nursing staff meetings.
  • Applies working knowledge of state, federal local and accreditation regulations for the delivery of hospice services.
  • Documents accurate assessment of patient status and updates the plan of care as needed in the EMR.
  • Documentation is completed at point of care and reflects interventions and response to care, focusing on patient’s goals.
  • Meets productivity expectations as set by the organization.
  • Other duties as assigned.

All staff are expected to participate as needed in variable shifts, including weekend rotation per staffing needs, departmental holiday rotation and follow the Comprehensive Emergency Management Grid during times of activated emergency situations.

Computer Skills: Strong computer skills including database software and general knowledge of email/outlook. Must be able to utilize device to document in Electronic Medical Record (EMR).

Supervisory Responsibilities: .This job supervises care provided by LPN and HHA's.

Education/Experience: Associates Degree in Nursing, Bachelor in Nursing (BSN) degree preferred. CHPN preferred. Hospice experience preferred.

Language Skills: Communication skills of reading, writing, and speaking in English.

Mathematical Skills: Math skills needed including addition, subtraction, multiplication, division and percentages/fractions with ability to calculate medication doses. Nurse must successfully pass the math test given at time of interview prior to starting orientation and annually.

Certificates, Licenses, Registrations: Current Registered Nurse License to practice in the state of Florida Healthcare Provider CPR required upon hire and maintained. CHPN preferred; Required within 2 years of employment. Valid Florida Driver's License required.

While performing the duties of this job, the employee is occasionally exposed to outside weather conditions. The noise level in the work environment is usually moderate. Visits outside of the Inpatient Unit setting involve driving to patient homes. Must be able to endure exposure to common household allergens (i.e., pets, plants, dust).
