• Conduct thorough assessments to understand the unique needs, strengths, and challenges faced by each client, ensuring that treatment plans are personalized and effective.
• Create and implement tailored therapeutic interventions that promote positive mental health outcomes and enhance clients' coping strategies.
• Maintain high standards of accurate and confidential documentation to adhere to ethical and legal guidelines concerning client privacy.
• Collaborate closely with a multidisciplinary treatment team, including physicians, nurses, and other healthcare professionals, to ensure a comprehensive approach to patient care.
• Continuously engage with ongoing training and research in social work methodologies to integrate the latest evidence-based practices into your work.

Your work will primarily focus on clients who are navigating the complexities of terminal illness, emotional distress, and the journey of end-of-life transitions. This sensitive environment requires an understanding of grief, loss, and the unique needs of hospice patients and their families. You will provide counsel, emotional support, and resources that empower clients to find peace and comfort during their remaining days.

• A Master’s degree in Social Work from an accredited institution is required.
• An active LCSW (Licensed Clinical Social Worker) license in the state of Illinois.
• Demonstrated excellence in communication and problem-solving skills.
• A passion for team collaboration and a strong ability to work effectively in a multidisciplinary environment.

• Prior experience in hospice, palliative care, or working with terminally ill patients is highly advantageous.
• Experience with telehealth or remote counseling platforms.
