Registered Nurse Diabetes Educator

Location: Rockford, Illinois

Pay Rate: $2,200 To $2,400/week

Contract: 13 weeks

Schedule: Mondayโ€“Friday, Day Shift

Total Working Hours: 40 Hours/Week

Experience: Minimum 2 years required

Setting: Clinical Diabetes Education and Nursing Care

About the Role

We are seeking an experienced Registered Nurse Diabetes Educator for a 13-week assignment in Rockford, Illinois. This role focuses on providing compassionate diabetes education, patient advocacy, and coordinated nursing care in collaboration with a multidisciplinary diabetes management team.

Responsibilities

โ€ข Provide diabetes education to patients and families living with diabetes mellitus

โ€ข Develop and support individualized patient care and education plans

โ€ข Collaborate with physicians, nurses, and interdisciplinary team members

โ€ข Administer medications and treatments according to nursing standards and policies

โ€ข Document care accurately in accordance with standards of care

โ€ข Serve as a resource for patient education materials, programs, and care guidance

โ€ข Support patient safety, patient experience, and performance improvement activities

Requirements

โ€ข Active Registered Nurse License in Illinois and Wisconsin Required

โ€ข Graduate of an accredited nursing program Required

โ€ข CDCES Certification Required

โ€ข BLS/BCLS Certification Required

โ€ข Minimum 2 years of nursing and/or diabetes education experience required

โ€ข Strong communication, patient education, and advocacy skills

โ€ข BSN Preferred

โ€ข EPIC experience Preferred
