Schedule Available: 8:00 AM to 4:30 PM, Monday through Fridayย ย 

Momentumย is currently hiring anย Adult Day Program Coordinatorย to join our team! The Adult Day Program Driver is responsible for providing transportation for people supported at Momentum locations in the San Fernando Valley to and from the Day Program in Thousand Oaks.ย ย 

In this dual hybrid role, in addition to providing transportation, this staff member will also work at the Day Program in Thousand Oaks.ย

ADP Coordinatorย Essential Responsibilities:ย 

  • Ensures safe operation of the residential transportation vehicles in accordance with Momentum's Operational Manual
  • Safely operates Momentum vehicles on surface streets, freeways, highways, and interstates in compliance with DMV and Highway Patrol protocols
  • Operates vehicle lift for people supported in a safe manner and ensures wheelchairs are secured properly. Safety policy requires a 4-point securement for all mobility devices
  • Assists with moving in and out of wheelchairs, automobiles, pool, etc.
  • Assists people attending the program with program activities and community-based activities
  • Assists people attending the program with communication, feeding, mobility, toileting, hygiene, and other activities of daily living
  • Engages in program activities including computer class, art class, sensory activities, and gardening
  • Performs other duties as assigned to meet department and/or business needs

ADPย Coordinatorย Education and Experience:

  • A high school diploma, GED, or equivalent; requiredย 
  • A minimum of two (2) years of related work experience; requiredย ย 
  • Experience working with individuals with developmental disabilities; preferredย 

Skills and Knowledge:ย 

  • Must be able to read, write, comprehend, and communicate effectively in English
  • Must have three (3) years driving experience
  • Ability to maintain confidentiality

Requirements:ย 

  • Valid and current proof of legal right to drive in California; required
  • Must have three (3) years driving experience; requiredย 
  • Current DMV driving record printout; required
  • Must be able to participate in pool therapy and aquatic activities
  • Successfully pass all post-offer, pre-employment screening requirements, including criminal background check, drug screening, physical exam, and provide proof of eligibility to work in the United States
  • Ability to run, push, pull, bend, squat, kneel, walk, stand and sit for long periods of time, and lift up to fifty (50) pounds
