Serenity Hospice PA is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our hospice team serving Dauphin and Lebanon Counties. This is a rewarding opportunity to provide high-quality, patient-centered care to individuals and families during an important stage of life.
Position Overview
The Hospice LPN delivers skilled nursing care under the direction of a Registered Nurse, supporting patients with comfort-focused care while ensuring dignity and quality of life.
Key Responsibilities
  • Provide direct patient care in accordance with individualized care plans
  • Administer medications and treatments as ordered
  • Monitor patient condition and report changes to the RN and care team
  • Educate patients and families on care needs and comfort measures
  • Maintain accurate and timely documentation
  • Collaborate with the interdisciplinary hospice team
Qualifications
  • Active LPN license in the state of Pennsylvania (required)
  • Previous hospice or home health experience preferred
