DIRECTOR OF REHAB: JOB DESCRIPTION
JOB REQUIREMENTS
● Bachelor of Arts, Science Degree, Master’s Degree, or an Associate Degree in an allied health
field, or comparable skilled nursing experience deemed adequate by Regional Director of
Operations.
● Current state licensure and/or certification/registry, as required.
PHYSICAL REQUIREMENTS
● Ability to sit for extended periods of time, walk through the department and office and lift objects
weighing up to ten pounds. Must be able to climb stairs, stoop and reach when performing daily
work.
● Must also meet physical requirements for job description of held licensure. For example, Physical
Therapist.

JOB RELATIONSHIPS
Responsible to and reports to the Director of Operations.
JOB RESPONSIBILITIES
1. Responsible for the management and administration of all aspects of rehabilitation services as well
as for maintaining a clinical caseload.
2. Responsible for recruiting and retaining all rehabilitation service personnel.
3. Ensures effective orientation and on-the-job training for all rehabilitation service personnel.
4. Promotes positive employee relations by facilitating an atmosphere of open communication and
problem solving.
5. Responsible for conducting performance reviews of all rehabilitation service personnel.
6. Responsible for all aspects of operations of rehabilitation services at the facility level.
7. Monitors personnel compliance with all standards and takes corrective action when needed.

8. Works with finance personnel in reconciling billing to ensure accuracy.
9. Assures that all program standards are met, including MDH SNF standards and any other overseeing
agencies.
10. Identifies program development needs of the community and implements appropriate programs.
11. Develops clinical affiliation programs with educational institutions.
12. Investigates, follows up and assists in the resolution of staff, service and resident-related issues as
requested by the Administrator and/or the Executive Director.
13. Reviews the quality and appropriateness of the total services delivered.
14. Attends facility meetings as requested.
15. Assesses resources and supplies available in the community.
16. Tracks MDS Assessment Period Data to include number of disciplines treating, treatment
days/week, and daily treatment time. Communicates data to appropriate community personnel.
17. Reviews resident specific PDPM category per assessment period. Coordinates change in MDS data
according to community procedure.
18. Responsible for utilizing tracking systems and for implementing changes that may be indicated.
19. Ensures compliance with data entry for resource utilization and CPT coding daily. Monitors
treatment minutes daily.
20. Assists with and participates in quality assurance programs as required.
21. Adheres to established confidentiality standards of the residents, as well as the organization.
22. Maintains appropriate relationships and rapport with coworkers, supervisors, residents, family
members and community personnel and administration.
23. Projects a positive and professional image at all times.
24. Generates special reports/data as needed to assist the Administrator and/or Executive Director and

facility.
26. Reports to work on time and coordinates schedule to achieve maximum results of Rehabilitative

complying with all Infection Control, Universal Precautions and OSHA standards for the healthcare
professional.
28. Performs such other duties as assigned and as required to effectively perform the responsibilities of
the position and are in the best interest of Northwoods Rehab.
