Position Summary

DaVita isseekinga strategic and execution-oriented leader to join the Home National Team as Director, Home Strategy & Operations. This highly visible leadership role will play a critical part in advancing DaVita's Home strategy by supporting growth across home dialysis modalities including Peritoneal Dialysis (PD) and Home Hemodialysis (HHD).

This leader will partner closely with field operations, clinical leadership, product teams, value-based care, finance, and physician partners to design, implement, and scale initiatives that improvepatient access toHomemodalities. Successrequiresa unique blend of strategy, project management, change leadership, stakeholder influence, and operational execution.

Key Responsibilities

Lead HomeConversionsStrategy & Performance Improvement

  • Lead and inspire teams focused on improvingteammate processes to educate patients and physicians onHomemodality options.

  • Partner with field leadership toidentifyopportunities to improve keyhome growth and clinical outcomes.

  • Develop and execute strategies that increasepatient access toHomemodalities.

  • Foster a culture of accountability, collaboration, and continuous improvement.

SupportNational Education Programs

  • Support Modality Choice Leads to prioritizetimely, quality patient education inall ofour centers.

  • Drive modality education program enhancements, includingleveraging digital capabilities and continuous improvement in EHR and other systems.

  • Drivepilot andadoption ofAI-driven predictive models.

  • Provide clinical and operationalexpertiseto improve patient management practices.

  • Establish processes, monitoring, and accountability structures to drive measurable results.

  • Identifyphysician, operational, and field barriers andfacilitatesolutions.

  • Evaluate pilot outcomes and scale successful practices nationally.

Clinical & Operational Innovation

  • Develop a standardized, data-driven approach forconversionsacross theHomebusiness.

  • Supportdevelopmentof educational content, training programs, and continuing education initiatives.

  • Serve as a key advisor for AI-powered patient-education initiatives.

Qualifications

Required

  • Bachelor'sdegreerequired.

  • 8+ years of healthcare strategy, operations, consulting, clinical operations, or related experience.

  • Experience leading enterprise-level programs with measurable outcomes.

  • Strong project management andchangemanagement capabilities.

  • Demonstrated success working within highly matrixed organizations.

  • Exceptional communication, influence, and stakeholder management skills.

  • Experience translating strategy into scalable execution plans.

Preferred

  • Home dialysis, kidney care, renal services, or specialty healthcare experience.

The Salary Range for the role is $115,000.00-$183,000.00 per year.
