Position Title
Hospice Intake & Staffing CoordinatorOlathe Medical Pavilion DPosition Summary / Career Interest:Home Health Admissions Coordinator is expected to support the mission, vision, and values of Olathe Medical Center, Inc. The major role of this position is to work collaboratively with all referral sources for the agency in providing transfer of patient care from any setting to the home-based services the agency provides.

Responsibilities and Essential Job Functions

  • Effectively coordinates patient admission process for Home Health department according to Home Care regulatory standards.
  • Coordinates scheduling for clinical field staff according to professional scope of practice, and Home Care regulatory standards. Consults Clinical Supervisors when indicated for assignment changes and productivity adherence.
  • Utilizes clinical expertise to determine appropriateness of patient referral for the Home Care setting.
  • Effectively communicates to clinicians, physicians, patients, patient care givers and referral sources adhering to departmental process and regulations for all forms of communication.
  • Accurately completes Home Health intake process to include:
    - Ensures referral information and orders are received.
    - Accurately completes intake documentation.
    - Verifies patient payer source.
    - Communication to assigned staff.
    - Completed referral is forwarded to the Data Department for processing.
  • Participates in department process activities to improve coordination of care between Home Health Department, hospital discharge planners and outside referral sources.
  • Effectively and appropriately supervises tasks delegated to clinical and non-clinical staff for accuracy."

Required Education and Experience

  • Graduation from an Accredited School of Nursing or Physical Therapy
  • 3 or more years of Experience in Home Care Industry

  • CPR/ AED/ BLS - Other BLS upon Hire
  • Licensed Registered Nurse (LRN) - Single State - State Board of Nursing OR
  • Licensed Physical Therapist (PT) - State Board of Healing Arts
