Job description

Primary Care NP – Make a Real Impact in Home and Community Medicine
Territory: St. Louis, MO
Join the team that’s redefining primary care.
Bloom Healthcare is an award-winning, employee-owned medical practice delivering high-touch, high-quality care to medically complex patients where they are—at home and in communities. Our innovative model consistently outperforms national benchmarks and has earned us “Top Workplace” honors six years running. We invest in our people first, because exceptional clinicians create exceptional patient outcomes.
If you’re a Nurse Practitioner or Physician Assistant who wants meaningful autonomy, manageable patient volumes, and a mission-driven culture that truly values you—Bloom is the place to build your career.

Why This Role Matters
As a Primary Care Provider, you’ll be a lifeline for homebound seniors, helping them age safely and comfortably in place. You will provide relationship-based primary care, respond to acute needs, and drive measurable improvements in health outcomes—all with full support from Bloom’s interdisciplinary team and advanced data systems.
This is the ideal role for a clinician who thrives on independence, enjoys complex primary care, and wants to practice medicine the way it was meant to be: patient-centered, unrushed, and deeply impactful.

What You’ll Do
  • Serve as the primary care provider for a defined patient panel, delivering longitudinal, holistic medical care in patient homes and communities.
  • Diagnose, manage, and treat chronic and acute conditions with support from our clinical operations, triage team, and MD leadership.
  • Address social determinants of health, including housing stability, nutrition, caregiver support, and access barriers.
  • Conduct specialty visits such as wound care, post-hospital transitional care, and condition-specific assessments.
  • Complete comprehensive evaluations including medication reconciliation, fall risk assessments, advance care planning, and health education for patients and caregivers.
  • Utilize Bloom’s integrated data ecosystem to track hospitalizations, coordinate timely post-discharge visits (within 72 hours), and drive superior outcomes.
  • Collaborate closely with internal and external care teams to ensure continuity, reduce readmissions, and stabilize high-risk patients.

What’s in It for You
A manageable, quality-focused schedule
  • Monday - Friday
  • 8-12 patients per day (no unrealistic volume expectations)
  • Call rotation
  • Weekends off and flexible daily workflows
Robust benefits – starting Day 1
  • $5,000 sign on bonus available
  • 100% employer-paid Health, Dental & Vision effective Day #1
  • 3 weeks PTO + 7 paid holidays
  • 401(k)
  • Employer-paid Basic Life & AD&D
  • CME allowance + paid licensing & malpractice
  • Strong support from MDs, triage, schedulers, IT, billing, and marketing—so you can focus on patient care
Meaningful autonomy + strong support
You practice independently, but never alone. Our interconnected care teams and advanced analytics ensure you always have what you need to make excellent clinical decisions.

You’ll Thrive Here If You Are…
  • A NP with a master’s degree and active Missouri licensure
  • Passionate about serving seniors and medically complex populations
  • Skilled at building trust, communicating clearly, and adapting to home-based environments
  • Motivated by mission, relationship-driven care, and meaningful outcomes
  • Ready to join a team that values your voice, expertise, and well-being
