Job description

Description

 UCP is seeking a passionate individual that is dedicated to help children with growth and education in daily living in their home and communities. They will work with families to create an individualized plans to support the specific needs and interests of each child. They will work with staff members and management to ensure that they are provided as much support as needed. With the addition of supporting BHPs in making any needed corrections. They are responsible for collecting data and reviewing weekly completed paperwork for treatment plans and goals to promote positive outcomes and changes. ESSENTIAL RESPONSIBILITIES

The Treatment Plan Coordinator shall have knowledge of each child he/she supports.

The Treatment Plan Coordinator shall develop each child's plan of care and oversee implementation of it in the home or community.

Provide supervision, support, and mentorship to home and community-based staff. 

Input data from BHP sessions in order to support the BCBA in making data-based treatment decisions.

Assure the service intake process is complete and timely.

Maintain organization of weekly progress notes, tally sheets, client data and other pertinent records.

Complete Mainecare authorization requests within the managed care system for sections 28 and 28 ABA.

Requirements

REQUIRED

Bachelor's Degree 

Valid State of Maine driver license, reliable transportation with proof of registration and insurance, and willingness to use own transportation for work.

Shall have or acquire and shall maintain First Aid/CPR/Blood borne pathogens, BHP and Safety Care certifications within the first 30 days of employment and ongoing, thereafter.

PREFERRED

Experience working with children who have an Autism Spectrum Diagnosis, experience working with families, and experience supervising staff.
