Job description

Home Telehealth RN Duties

The Home Telehealth RN assumes the following major duties and responsibilities:

  • Conducts thorough initial and ongoing assessments, encompassing a review of the Veteran's overall health status, to develop a personalized plan of care that addresses their unique needs and goals.
  • Uses the treatment plan to assess, identify, analyze, and prioritize problems, interventions, and SMART goals (Simple, Measurable, Achievable, Realistic, Time-oriented).
  • Triage and assess all data received from Home Telehealth patients such as vital signs, reported symptoms, and question responses.
  • Reviews all patient responses each business day and contacts patients with high-risk responses and trends, significant changes in condition or changes in other specific data elements received as clinically appropriate.
  • Identify and intervenes for potential exacerbations or complications to facilitate timely care in clinic, emergency response/urgent care, or care in the community.
  • Provide appropriate interventions such as medication management, case management, health coaching, and patient education.
  • Triage incoming calls and concerns of patients/families, resolve those within scope of practice and route to other interdisciplinary team staff or services. Provide interdisciplinary consultations and interventions with cardiology, endocrine, pulmonary, renal, HBPC, and other non-institutional care programs/services such as mental health, nutrition, pharmacy, social work, and prosthetics.
  • Identifies patient knowledge, health factors, skills and behaviors that support self-management.
  • Document and communicate with PACT and Specialty Clinics providers/teams regarding changes in status: progress toward goals; patterns or trends in data: symptoms or findings of concern and need for provider assessment and/or interventions.
  • Facilitate, document, and communicate treatment changes to the Veterans as directed by providers. Provides follow-up evaluation of the Veteran after changes are implemented.
  • Demonstrates leadership in advancing telehealth technologies and innovations.
  • Demonstrates initiative to actively participate as a representative of the RPM-HT in PACT monthly meetings.
  • Provides leadership in performance improvement activities, implementation of new telehealth program initiatives or strategic goals, and Nursing Service level activities. Maintains home telehealth program initiatives and strategic goals
  • Serves as a representative on local, VISN, or National Office of Connected Care committees or work groups.
  • Leads and actively participates in educational initiatives for RPM-HT staff to improve Veteran outcomes.
  • Precepts new employees to the RPM-HT program.
  • Performs other duties as assigned.
