This is an excellent opportunity for a nurse who is passionate about providing hands-on patient care while supporting individuals and families through every stage of the hospice journey.

Key Responsibilities
• Provide skilled nursing care under the direction of the Registered Nurse while following each patient's individualized plan of care.
• Monitor patient conditions, recognize changes in health status, and communicate clinical updates to the care team.
• Assist with symptom management, medication administration, treatments, and patient comfort measures.
• Complete accurate and timely documentation of patient care and observations within the electronic medical record (EMR).
• Collaborate with physicians, Registered Nurses, caregivers, and the interdisciplinary team to deliver compassionate, high-quality hospice care.
Qualifications
• Active Licensed Practical Nurse (LPN) license
• Previous nursing experience required; hospice, home health, long-term care, or skilled nursing experience preferred.
• Strong clinical assessment, communication, and patient care skills.
• Ability to work independently while effectively collaborating with an interdisciplinary team.
• Experience with electronic medical records (EMR) preferred.
Field Role Requirements
• Valid driver's license, reliable transportation, and current automobile insurance.
• Ability to pass a criminal background check and Motor Vehicle Record (MVR) screening.
• Current CPR certification.
• Current TB testing in accordance with agency policy.
