$20/HR. + $1,000 Sign on Bonus

3 day shifts a week 7 am - 7 pm; including every other weekend

Creative living group focuses on providing custom support services designed especially for the individuals that we serve. We provide comprehensive training to help you build your skills and experience.Who are we looking for?

We currently are looking for an experienced, caring, and reliable Direct Care Professional with Program and Designated Coordinator management experience.

· You will help the person served, who experiences seizures, with all of their daily living skills, personal cares, and social/emotional development

· Provide support to the Program Manager to ensure policies and procedures are being followed at all times

· Must possess the ability to plan, organize, develop, implement and interpret the programs, goals, objectives, policies, and procedures that are necessary for providing quality care

· Must possess leadership and supervisory ability and communicate well with all team members

· Possess good computer skills with Word and Excel

· Participate in individual program planning and implementation to help the person attain their personal life goals

· Help with staff training and ensure that all training documentation is completed and submitted before their first shift alone

· Manage and coordinate staff schedule

· Oversee that all documentation is completed accurately and up to date

· Manage that all shifts are covered and flexible to fill in as needed

· Available to take calls from staff outside of own scheduled shifts

· Perform Designated Coordinator Responsibilities

· Must demonstrate competency in the completion of the above-listed job responsibilities

· Ability to effectively communicate in English, both writing and verbally.

Educational and Licensing requirements:

· Must be 21 years or older

· Be able to pass a DHS background study

· Have a valid driver’s license and good driving record

· Required to be Designated Coordinator qualified, and must be met through one of the following:

(1) a baccalaureate degree (BA) in a field related to human services, and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; or

(2) an associate degree (AA) in a field related to human services, and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; or

(3)a diploma in a field related to human services from an accredited post-secondary institution and three years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; or

(4) a minimum of 50 hours of education and training related to human services and disabilities; and four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the qualifications identified in clauses (1) to (3) or

(5) a minimum of three years of supervisory level experience in a program providing direct support services to persons with disabilities or persons age 65 and older
