• Conduct comprehensive mental health evaluations, inclusive of psychosocial assessments, diagnostic interviews, and risk assessments to determine the needs of patients entering hospice care.
• Collaborate with patients to create individualized treatment plans based on their specific needs, goals, and cultural considerations to promote optimal outcomes during their end-of-life care.
• Provide therapy in individual, couple, family, and group settings utilizing proven therapeutic strategies such as CBT, DBT, and Solution-Focused Brief Therapy (SFBT).
• Deliver crisis intervention services, working closely with other healthcare professionals to ensure continuity of care for patients experiencing acute distress or emotional crises.
• Maintain meticulous clinical records, including progress notes, treatment plans, and discharge summaries, ensuring adherence to clinic policies and regulatory standards.
• Engage in psychoeducation for patients and their families, equipping them with knowledge and coping strategies to effectively manage mental health challenges associated with terminal illness.
• Participate in multidisciplinary team meetings with psychiatrists, psychologists, nurses, and case managers to coordinate comprehensive patient care.
• Conduct regular evaluations of treatment effectiveness and make necessary adjustments, ensuring the therapeutic approach remains aligned with each patient’s evolving needs.
• Stay informed about the latest research, best practices, and emerging trends in mental health, integrating this knowledge into daily clinical practice.
• Adhere to ethical guidelines, ensuring patient confidentiality and the safeguarding of sensitive information at all times.

You will primarily work with patients in hospice care, who may experience significant emotional and psychological challenges related to end-of-life issues. Understanding their unique situations and empathizing with their experiences is crucial for providing effective support. Your role will be instrumental in enhancing their mental wellbeing as they navigate this pivotal stage of their lives.

• Master's degree in Social Work (MSW) is required.
• Must possess an active State license as a Licensed Clinical Social Worker (LCSW).
• Demonstrated ability to work both independently and collaboratively within a multidisciplinary care team.
• Exceptional interpersonal and communication skills, connecting effectively and establishing rapport with diverse patients.

• Experience in hospice or end-of-life care settings is highly desirable, providing insight into the unique challenges faced by patients in this population.
• Proficiency in providing trauma-informed care and utilizing various therapeutic models strongly preferred.
• Experience coordinating care within a multidisciplinary team is an advantage.

• Annual base salary range is competitive, set between $80,000 and $100,000, commensurate with experience and skills.
