Job Details:

Candidate Type: Traveler

Requirements: RN License; BLS; PALS; ACLS; 1-2 years current experience as a Registered Nurse in a clinical care setting; 2+ years Home health experience; Proof of current CPR certification; Reliable transportation and insurance

Responsibilities:

  • Assess home health patients to identify physical, psychosocial, and environmental needs.
  • Develop, implement, and evaluate the Plan of Care to ensure quality and continuity of care.
  • Educate patients and families regarding care, medications, and safety procedures.
  • Supervise and provide clinical directions to home health aides, LVNs, and other staff.
  • Monitor and assess pain, utilizing appropriate pain management techniques and education.
  • Coordinate referrals to appropriate agencies as needed.
  • Perform on-call responsibilities and provide services to patients and families.
  • Communicate effectively with patients, families, and healthcare team members.
  • Attend interdisciplinary team and related meetings as necessary.
  • Ensure documentation is accurate, complete, and compliant with agency and third-party requirements.
