Job description

Patient Enrollment Specialist

This position will be located onsite within Hannibal Regional Healthcare System. Responsible for patient enrollment services associated with the digital remote patient monitoring (RPM) programs (Chronic diseases like but not limited to, Heart Failure, COPD, Diabetes). Includes digital platform enrollment and alert monitoring, patient outreach and education, coordination and escalation of care, and communication and collaboration with the providers and multidisciplinary team. Responsible for patient enrollment services associated with Chronic Care Management, including patient outreach and education. LPN or CMA/RMA required. Minimum of 2 years clinical experience required. Care management experience preferred and/or willingness to become certified in care management. Strong verbal and written communication and facilitation skills required. Strong assessment skills, time management skills, critical thinking skills required. Advanced computer skills required. Must have ability to interact effectively with a variety of people and situations at all levels of the organization.
