We are currently seeking a full time Patient Care Coordinator at our clinic in Cook. This position will manage the patient referral and consultation processes for the Cook medical clinic patients. This position will work with medical providers to ensure information is complete and properly routed to patient charts in the Electronic Medical Records system (EMR) as well as processing patient referrals accurately. The position will also be cross-trained to perform additional medical records as well as back-up support to the Medical Receptionist. Hours are Monday through Friday, 8am-4:30pm or as designated.
Patient Care Coordinator Responsibilities:
- Complete all order referral requests
- Set up medical patient appointments with other medical facilities for physician consultations
- Gather required notes from patient charts in the Electronic Medical Records (EMR) system and send to appropriate medical facilities for consultations
- Collect consultation reports and ensure documentation is sent back in a timely manner from medical facilities
- Scan and route necessary patient health information to providers utilizing the EMR
- Serve as primary back up for Cook medical site medical records department
- Scan documents into EMR
- Manage incoming provider orders, forms and miscellaneous outside requests needed by providers
- Assist as a backup to Medical Reception such as answering patient phone calls, conducting patient registration and scheduling appointments
- Complete Prior Authorizations
- Complete Insurance Referrals
- Miscellaneous administrative duties as assigned
