Job description

SAI Residential Treatment Center is seeking an experienced and compassionate Registered Nurse (RN) to provide direct nursing care to adolescents in a residential behavioral health setting.

The RN is responsible for assessing and monitoring residents' physical and mental health needs, administering and managing medications, maintaining accurate clinical documentation, and collaborating with the medical, clinical, and residential teams.

This is a direct-care nursing position and does not include supervision or management of nursing staff. The RN works under the direction of the Charge Nurse and in collaboration with the program physician and multidisciplinary treatment team.

Key Responsibilities

  • Provide direct nursing care to residents in accordance with established policies, physician orders, treatment plans, and applicable regulatory requirements.
  • Complete nursing assessments and monitor residents for changes in physical, behavioral, and mental health status.
  • Verify, count, prepare, and administer medications according to physician orders and established medication administration procedures.
  • Monitor residents for medication side effects, adverse reactions, effectiveness, and potential drug interactions.
  • Educate residents regarding prescribed medications, including common side effects and medication-related precautions.
  • Ensure resident prescriptions are filled and an appropriate medication supply is maintained throughout the resident's stay.
  • Monitor medication administration records and medication logbooks for completeness and accuracy.
  • Immediately report medication discrepancies, errors, adverse reactions, or significant health concerns according to facility procedures.
  • Work collaboratively with the program physician regarding physical health concerns, laboratory results, medication-related concerns, and recommendations for resident medical care.
  • Participate in the development, implementation, and evaluation of individualized nursing plans based on nursing assessments and identified resident needs.
  • Implement nursing components of each resident's multidisciplinary treatment plan.
  • Document nursing care, assessments, observations, interventions, medication administration, and resident responses accurately and within required timeframes.
  • Report significant changes in a resident's condition to the appropriate medical provider, Charge Nurse, and members of the treatment team.
  • Participate in multidisciplinary treatment team meetings as needed and communicate relevant medical and nursing concerns.
  • Provide nursing support during behavioral or medical crises and participate in post-incident debriefings when appropriate.
  • Maintain established infection prevention and control, health, safety, and environmental procedures.
  • Assist with maintaining appropriate medication, medical supply, and equipment inventory.
  • Utilize the facility's electronic health record and other designated systems to maintain accurate resident health information.
  • Maintain resident confidentiality and comply with HIPAA and all applicable privacy requirements.
  • Follow facility policies and applicable accreditation, licensing, and regulatory requirements.
  • Identify and report potential compliance, safety, documentation, or resident-care concerns to the Charge Nurse or appropriate leadership.
  • Maintain professional boundaries and therapeutic communication with residents and families.
  • Collaborate with clinical, medical, residential, and administrative staff to support continuity and coordination of care.
  • Participate in required trainings, competency assessments, quality improvement activities, and nursing meetings.
  • Perform other nursing duties within the RN's scope of practice as assigned.

Qualifications

  • Current Registered Nurse (RN) license in the State of Nevada in good standing.
  • Psychiatric, behavioral health, residential treatment, or adolescent nursing experience preferred.
  • Experience working with adolescents with behavioral or mental health needs strongly preferred.
  • Knowledge of medication administration, nursing assessment, infection control, and clinical documentation.
  • Familiarity with behavioral health regulatory and accreditation requirements preferred.
  • Ability to recognize changes in resident condition and appropriately escalate medical or behavioral health concerns.
  • Strong clinical judgment, critical-thinking, communication, and organizational skills.
  • Ability to remain calm, professional, and trauma-informed during high-stress or crisis situations.
  • Ability to work effectively as part of a multidisciplinary treatment team.
