Key Responsibilities

Your day-to-day responsibilities will include a variety of tasks aimed at promoting recovery and well-being among our patients. Key responsibilities involve:

• Conducting comprehensive psychosocial assessments to evaluate patients' emotional, social, and environmental factors that may influence their recovery journey.
• Providing patient and family counseling aimed at addressing adjustment issues related to rehabilitation and emotional challenges.
• Collaborating with an interdisciplinary rehabilitation team to develop individualized treatment plans that align with the best practices in the field.
• Assisting patients in accessing vital community resources such as support groups, vocational rehabilitation opportunities, and housing assistance.
• Engaging in ongoing education and support with patients and families about available post-rehabilitation services and community resources.
• Participating in discharge planning processes, ensuring smooth transitions as patients move into home settings or other care environments.

Patient Population / Client Focus

You will be working with a diverse patient demographic, primarily focusing on individuals in detoxification and rehabilitation stages. Qualifications

The ideal candidate for this position will possess the following qualifications:

• A Master’s degree in Social Work from an accredited institution.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
