About the Role:

As a Contract LPN/RN, you will play a vital role in delivering exceptional patient care while building strong relationships with our residents and their families. Contract LPN/RN will work closely with our interdisciplinary team to develop and implement comprehensive care plans, assess patient needs, and provide medications and treatments as prescribed by physicians. The Contract LPN/RN expertise will also be used to monitor patient conditions, report changes to the healthcare team, and maintain accurate and timely records.

Key Responsibilities:

• Administer medications and treatments as prescribed by physicians
• Provide nursing care to residents, including wound care, vital sign monitoring, and assistance with activities of daily living (ADLs)
• Develop and implement comprehensive care plans in collaboration with the interdisciplinary team
• Conduct patient assessments, identify areas of concern, and report changes to the healthcare team
• Maintain accurate and timely records of patient care
• Collaborate with colleagues to ensure smooth delivery of care services
• Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
